Patient Information
At your first visit to our office, we will ask you to complete a comprehensive health history form which the dentist or hygienist will review with you. Depending on your age, risks of disease and symptoms, the dentist may recommend x-rays. The dentist will perform a thorough examination of your teeth, gums, and mouth looking for any signs of disease or other concerns.
Our goal is to help you maintain good oral health and to prevent any problems from becoming serious by identifying and treating them as soon as possible.
To schedule an appointment, please call 603-788-2517.
Please download our new patient form. This form can be filled out digitally and e-mailed back to us at info@lancasterdentalcarenh.com or print it out and bring the completed form in before your first visit.
If you would like to read and sign our HIPPA form prior to coming into the office, please download, print and fill out our HIPPA form. Or you can receive a copy in our office and can sign it at that time.
Why We Use X-Rays
Radiographic (x-ray) examinations provide the dentist with an important tool that shows the condition of your teeth, presence or degree of periodontal (gum) disease, abscesses and abnormal growths such as cysts or tumors. X-rays also show the exact location of impacted and unerupted teeth and pinpoint the location of cavities and other signs of disease that may not be possible to detect through a visual examination.
Insurance
Our dentists at Lancaster Dental Care are Northeast Delta Dental Providers and New Hampshire Healthy Kids Medicaid Providers. We also accept CareCredit®.
Patient Testimonials
I have been visiting the Lancaster Dental Care office for my entire life. I have had an excellent experience throughout my “teeth care” journey. More recently, I faced a serious deterioration of my gum line. Dr. Schmidt and his team have been right there by my side monitoring this issue with vigilance. The team at Lancaster Dental Care makes you feel like family and they would do anything possible to maintain your dental health, as well as, attentive to how you are doing as a person. We are blessed to have this level of dental care in the North Country.

Lancaster Dental Care has been mine and my husband’s dental office from childhood. I have lovely memories of trips in with my whole family, including elderly grandparents. Now…with our own children, we continue to love the whole experience of going to the dentist. All five of us! We value our relationships with the hygienists, the front desk staff and Dr. Schmidt himself and every visit not only maintains our dental health, but is always a positive social experience for our family. We cannot recommend Lancaster Dental Care enough.

For the past 20 years I have been going to the dentist in Canada. Because of the border shut down I now had to find another dentist as I had a tooth that was giving me problems. The dentist I saw was Dr. Corey Wells. My first appointment with him was very pleasurable. He was professional and very thorough on explaining the procedure he was going to perform on my next visit. I am looking forward to my next visit and will definitely continue with Dr. Wells and the Lancaster Dental Care professionals. All the staff were friendly and made you feel at home.

We are so fortunate to have Dr. Corey Wells and his family settle here and establish a dental practice. He recently performed extensive and high-quality dental restoration work on me. Could not be more pleased. He is obviously well trained and highly skilled.
